The balk rule

The balk rule
I have been watching baseball for a long time now, and I am embarassed to say that I don't completely understand the balk rule. I know there is some difference in the rule depending on where the runner is on the bases. The only thing I know is that the pitcher's foot has to be off the rubber and pointing towards the base being thrown to, but I am not too sure. Can anyone explain to me exactly what the rule is?

Thanks.
